When you test positive or are a Close Contact for COVID-19, you must declare this using our Employee COVID Declaration.
Who should use the declaration form?
- Employees with a positive PCR or RAT test
- Employees with a close contact to report (4 hours or more with a COVID case)
Can I still call the hotline?
Yes, you are still welcome to call the hotline – however if you’re calling to report a close contact or positive test, you should do so using the Employee COVID Declaration form.
