Disability Liaison Officers – reducing barriers to healthcare for people with a disability

Under our Disability Action Plan, Monash Health commits to eliminating barriers that prevent people from accessing and participating in its services.

For our consumers with a disability, Monash Health employees can offer the services of a Disability Liaison Officer (DLO) who can provide support to ensure people with a disability can access the healthcare they require.

Referrals to DLOs can now be made through the EMR referral process. Simply select the Disability Liaison Program that appears in EMR. See the Ordering a Consult / Referral QRG for more information.

Referrals to the Disability Liaison Program can also be made by phone or email – see the DLO information sheet for details.

How can a DLO assist?

DLOs are registered nurses who can assist with the coordination of healthcare for people with disabilities. DLOs work across the life span in paediatrics and adult services. The approach of the DLO program is to work in partnership with teams and population-specific services such as Aboriginal Health and Refugee Health.

Specifically, DLOs can:

  • Assist and support people with a disability to ensure adjustments related to their disability are made in their health care delivery.
  • Provide advocacy and support to people with a disability accessing Monash Health service, including communicating with their health care professionals.
  • Provide support with discharge planning from hospital and accessing community-based services, including NDIS.
  • Support adults with a disability, their families and carers, to access COVID-19 screening, treatment and vaccination.

If you would like to know more about DLOs, including how you can connect your patients with DLO services, please email the team, or see the DLO information sheet.
