For existing Monash Health patients
All referrals for Cardiology and Cardiac Surgery clinics, and any Cardiac diagnostic tests are now done in the EMR.
The Ambulatory and Outpatient Cardiac Clinic Referrals QRG shows doctors how to locate Ambulatory/Outpatient Cardiac Clinic Referrals in the EMR.
For non-Monash Health patients
For patients without an existing Monash Health UR number who require an outpatient cardiac test or clinic appointment this can be placed in HealthLink. The Placing a Monash Heart HealthLink Referral QRG will guide you through the steps for making these referrals. Please note if you are not registered on HealthLink there is a 24-to-48-hour turnaround from the time you register to being able to make referrals
Triaging of Cardiology and Cardiac Surgery Clinic Referrals
If you are a clinician triaging cardiology or cardiac surgery referrals, and you find an SMR eReferral, please email the UR to Referral Management for manual processing.
