At 8.00 am on Monday 20 November, Monash Health TCM (The Care Manager) users will formally transition to the Health Management System (HMS) for community-based client management.
About the Health Management System
The HMS will be our go-to client management system to manage patients receiving community-based care. This includes services delivered in the home, in residential aged care facilities and in other community settings. It will function as a client-management, referral and brokered services system.
In the HMS system, our brokerage rates will be managed centrally, providing for an easier and more uniform approach. We will also have enhanced visibility of data via the Business Intelligence (BI) portal, giving us access to meaningful data in an easier way.
Key goals
The change represents an important first step in reviewing our processes to ensure we have effective and intuitive systems that support our teams in the best way possible.
The transition will also allow for ongoing compliance with government regulations, reporting standards, and statutory data requirements.
Following the transition to the HMS, we look forward to introducing new updates and features over time, and to hearing from our teams on their needs and experiences so we can continue to improve.

- Monash Children’s Hospital @ Home Post-Acute Care
- Transition Care Program
- Post-Acute Care (Adult)
- Residential In Reach
