COVID-19 Model of Care

As part of our COVID-19 response, we have adapted our Model of Care.

We have drawn on our expertise and the lessons learned to refine our Model of Care – ensuring our workforce is best placed to care for COVID positive patients without compromising safety or patient outcomes. As always, maintaining the safety of our employees, patients, and community remains at the core of every decision.

COVID-19 Clearance

If you have a patient that requires COVID-19 clearance, please download our COVID-19 clearance form and email the completed form to covidclearance@monashhealth.org.

COVID clearance emails will be checked regularly, including weekends, so please don’t contact Infection Prevention directly with your COVID-19 clearance enquiries.

Community Fast Track Response Services

Active from 1 February 2022
Most Monash Health Community services are operating as escalation services under Code Brown via the Fast Track pathway.

Community Fast Track provides a centralised 7 day a week structure to coordinate workflow and allocation of resources within Community.

The purpose of Fast Track Services is to:

  • provide a single point of reference and streamlined referral pathway
  • ​​​​​divert care from ED to best clinical or social support pathway
  • identify vulnerable and high-risk complex patients that can be treated by Community​
  • provide responsive and flexible care as close to the person’s home as possible
  • facilitate early discharge from acute hospitals

High Acuity Discharge Clinic (HADC)

Monash Health is running a High Acuity Discharge Clinic (HADC) at Clinic M, Monash Medical Centre, Clayton.

The High Acuity Discharge Clinic (HADC) provides some of our non-COVID patients a quick follow-up process/review and enable them to wait for tests and/or resolution of illness at home. It will also increase our inpatient/bed capacity and support our increased role in caring for COVID-19 positive patients.
