Give Me Five conversations support regular, meaningful discussions between employees and their managers about contributions at work and future aspirations.

These conversations are a chance for employees to be empowered, engaged, and involved in decisions about their role and future goals and objectives. It’s also a time to highlight progress from previous conversations and growth opportunities.

Recording Give Me Five discussions

It is important that we keep a records of when the discussions have taken place. Use one of the following (agreed upon) methods for recording discussions.

By the manager

  1. Download and complete the Performance Review Record spreadsheet.
  2. Email the completed spreadsheet to: performancereviews@monashhealth.org

By the employee

Upon agreement with their manager, a record of a completed Give Me Five/Performance Review conversation can be added directly to HR 21/Employee Self Service by individual employees.

Executive Director/GM/Director level

Employees at the Executive Director/GM/Director level are required to complete the Executive Performance Enhancement plan as part of their performance review process.
