What classifies as another organisation?
This includes any other organisation you work for, in a paid, unpaid or voluntary capacity. For example; these may include supermarkets, pharmacies, retail stores, hospitality services, sports centres, educational institutions, other health services or community centres.
I have already commenced my placement at Monash Health; do I still need to declare?
Yes. You must complete the form as soon as possible.
I volunteer at another organisation; do I need to declare this?
Yes, any unpaid or voluntary employment must be declared when completing the form.
My other job is not at a health service, do I still need to declare this?
Yes, as we know, workplaces have been one of the major places of coronavirus transmission. Having this information helps us to efficiently plan our response and will assist us with the contact tracing process.
I work for another employer, but I work for them from home. Do I need to declare?
Yes, you do need to declare this. Please advise us of all employers, irrespective of location.
What if I start or stop working for an organisation during my placement?
If your current paid, unpaid or voluntary work arrangements change, please inform your relevant student coordinator.
I do not work or volunteer for any organisation; do I still need to declare?
Yes. Please declare that you do not work for any other organisation in a paid, unpaid or voluntary capacity.
Why do we require this information?
This form relates to a Directive from the Public Health Commander, under the Public Health and Wellbeing Act 2008. The information collected will help inform the Department’s response to the COVID-19 pandemic, including outbreaks in healthcare settings.
