Congratulations to our Rehabilitation and Aged Care team on Ward H at Casey Hospital for achieving the best results in the state in the Safer Care Victoria Delirium Collaborative.
Safer Care Victoria has partnered with the Institute of Healthcare Improvement to assist 21 health services in a Breakthrough Series Delirium Collaboration to improve the prevention and management of delirium.
Our staff on Ward H have implemented different strategies, including screening, management and environmental audits, to improve care for our patients with a cognitive impairment.
Since using the 4AT to screen for delirium, we have seen a reduction in falls, and a decrease in the use of antipsychotics for patients with delirium.
The number of patients completing a Sunflower and TOP 5 tool to develop individualised care plans has also increased, and our staff, patients and families have reported that their overall experience has improved.
