Clinical Transition Team ready for Go-Live 1

With an important job ahead, our Electronic Medical Record (EMR) clinical transition teams have successfully completed training and are now ready for Go-Live 1.

At Go-Live, clinical transition teams will be responsible for transferring inpatient medical information from paper files into the EMR.

Each clinical transition team is made up of a pharmacist, doctor and nurse. The team will take the paper file and work in a designated room, within a robust framework, working with and referring to the Nurse Manager. You’ll see safety collateral (wristbands, A4 signage and stickers) at the bedside, communicating where each patient’s transition is up to.

The training covered everything from the overarching Clinical Transition Principles and rules for Go-Live, a ‘how-to’ guide of how to transcribe patient charts, enhancing communication between teams and how to escalate issues.

We will be providing patients involved in clinical transition with a comprehensive suite of information, which will be distributed to operational areas from 19 August.
