If you have received your flu vaccination outside of Monash Health, you must declare it by completing the online Flu Vaccination Declaration form.
With many of us working remotely, you may have opted to get your flu vaccination closer to home.
It is important that you fill out the online declaration form to tell us that you have received your flu vaccination elsewhere this year. If you are not participating, you also need to fill out the form.
If you have not received your flu vaccination, we encourage you to do so by attending an Infection Prevention office. Times and locations can be found on the Monash Health COVID-19 website.
