Advance Care Planning supports patients in the shared decision-making process. It allows them to:
- appoint a support person to assist them in making decisions while they have capacity,
- appoint a medical treatment decision-maker to make decisions for them if they have a loss of decision-making capacity, and,
- document their future preferences regarding medical care in an advance care directive.
