The Pharmacy View in the EMR is being updated and implemented across our sites.
The new Pharmacy View contains interactive components that enable pharmacists to document pharmacy-related documentation in the patient record, whilst viewing the patient’s information within one view.
Last week, we began making these updates and improvements based on the following sites and dates:
- Kingston – Tuesday, 9 November
- Moorabbin – Thursday, 11 November
- Clayton – Tuesday, 16 November
- Dandenong – Wednesday, 17 December
- Casey and Cranbourne – Thursday, 18 November
To learn about the new Pharmacy documentation processes, refer to the appropriate Quick Reference Guide and Key Workflow videos that are outlined in the Pharmacy EMR Passport.
The Pharmacy View update improves both documentation options and visibility for documenting and completing a number of EMR fields, including:
Pharmacy Admission Histories
Admission History will now be in its own dedicated note type and much easier to read than the previous text rendered version of PowerForm. The treating team and multi-disciplinary team can view documented pharmacy admission histories in the patient’s ‘Documents’.
Pharmacy Inpatient Medication Management Plans
Inpatient medication reviews conducted by a pharmacist will be in a note rather than a PowerForm. The treating team and multi-disciplinary team can view documented pharmacy Inpatient Medication Management Plan in the patient’s ‘Documents’.
Pharmacy Internal Weekday Clinical Handovers
Pharmacy Internal Weekday Clinical Handovers will now be documented in a dedicated PowerForm. If relevant and applicable, the treating team and multi-disciplinary team can view documented handover PowerForm in the ‘Form Browser’ of the patient record.
Pharmacy Clinical Intervention Forms
Clinical Interventions PowerForms will now be text-rendered, even if it has not been resolved. The treating team and multi-disciplinary team can view documented Pharmacy Clinical Interventions in the patient’s ‘Documents’ or updated from the ‘Form Browser’ of the patient’s record.
Pharmacy Discharge Medication Management Plan PowerForm
The Pharmacy Discharge Medication Management Plan PowerForm will now be in its own dedicated form and much easier to read and document. The treating team and multi-disciplinary team can view documented Pharmacy Discharge Medication Management Plans in the ‘Form Browser’ of the patient’s record.
