We have made changes to our Employee COVID Declaration form to capture Close Contact reporting as well as positive cases.
Who should use the declaration form?
- Employees with a positive PCR or RAT test
- Employees with a close contact to report (4 hours or more with a COVID case)
Can I still call the hotline?
Yes, you are still welcome to call the hotline – however if you’re calling to report a close contact or positive test, you should now do so using the Employee COVID Declaration form.
