Since 2009 the Transition Care Program (TCP) has operated a bed-based service across South 1, Kingston Centre and from Homewood, TLC. Forty-eight beds will now operate from Regis Dandenong North and Regis Cranbourne, with 30 home-based beds remaining as is.
We would like to thank both the TCP and the South 1 teams for their long and successful partnership in delivering care to some of our most vulnerable community members.
South 1 is in the process of being recommissioned as a 20 beds Geriatric Evaluation & Management (GEM) ward, with details currently being finalised.

Are there any changes to the TCP referral process?

About TCP
The Transition Care Program is a short-term program providing care and assistance with recovery for older people who have been in hospital. By offering low level therapy and case management support, the program allows people to be discharged quicker and continue their recovery in a non-hospital environment, while long term care is arranged. Services can be provided as either home based or bed-based, within a Residential Aged Care setting. The time on the program is dependent on individual care goals, however the limit is twelve weeks.
Eligibility
To access TCP patients must:
- be medically stable and have completed their acute and/or sub-acute hospital stay
- be able to make some functional gains or benefit from a period of active management to complete the restorative process
- have a current TCP approval from the Aged Care Assessment Service (ACAS)
- wish to enter the program
- have a Medicare card
