The email address will be de-activated from Monday 30 January 2023. Please send all community-based referrals to icareaccess@monashhealth.org in accordance with the internal referral guide.
The code brown referral form to Community-based services and cb.communityreferrals@monashhealth.org email address was introduced during the code brown response in January 2022.
