What’s happening?
From Monday 3 April 2023, Monash Health will upgrade from RiskMan version VHIMS1 (Victorian Health Incident Management System) to the RiskMan version VHIMS2.
What is it, and who does it apply to?
Under the requirements from the Victorian Agency for Health Information (VAHI), all health services are required to capture incidents across three categories using a new Minimum Data Set (MDS), contained in VHIMS2. The incident categories are:
- Patient/clinical incidents
- Employee/OHS incidents
- Hazard incidents
All staff members need to be aware that categorising the type of event in an incident report (patient/employee/hazard) will look slightly different in RiskMan version VHIMS2.
The upgrade also aims to simplify and make existing reporting of patient/employee/hazard safety incidents easier and quicker to document.
All existing incident reports entered in VHIMS1 will still be accessible following the upgrade.

- Live demonstration and a “first look” into the system, including quick tips on entering an incident
- What the main changes are
- Opportunity for Q&A

To report an incident, please click on the RiskMan application
found on your Desktop screen, which will take you to Monash Health’s RiskMan Site. You are able to log into RiskMan using your network login and password.
