Since we launched our Fitness Passport sign up campaign, we’ve had some great feedback and questions about the program.
Here are the answers to some of your frequently asked questions.

We’re excited too, but we need 1000 people to sign up by 5 May for the program to activate!
How can I sign up?
Visit the Fitness Passport portal to register for your membership.
Can I join Fitness Passport later?
You can! But only if we hit our 1000-person target to activate the program. Between now and 5 May is the best time to register for Fitness Passport to give us the best chance of the program going ahead.
Where can I find the list of facilities included with Fitness Passport?

Is there a joining fee?
There is an initial card processing fee payable once your membership has been approved by your workplace facilitator. This cost is one fortnight’s membership fee and is payable prior to receiving a membership card.
How much does Fitness Passport cost?

Is the cost of the Fitness Passport debited from your salary before tax?
No. Like most gym memberships, your Fitness Passport is paid fortnightly via direct debit from your bank account.
When can I start going to the gym?
If we hit our target of 1000 people by 5 May, we can expect the first direct debit to occur within the fortnight, and Fitness Passport access cards to be mailed out within one fortnight of the first direct debit.
As soon as we hit our target, we’ll be in touch with confirmed dates.
Does the Fitness Passport give 24/7 access to facilities that operate 24/7?
Yes! But you will have to attend those 24/7 facilities during staffed hours to obtain a fob or access card for that specific facility.
Do I have to pay for access cards/fobs at every facility?
A number of facilities request members wishing to access their facility to purchase a swipe key so you can gain access to the facility. This depends on the facility you attend but if it is required, it is a one-time payment made directly to the gym. This key may also be used to keep a record of visitation so must be purchased whether you intend to use the gym outside of staffed hours or not. Fees may vary depending on the location.
You can view any additional fees required by gyms by logging in to your account and selecting the facility on the map. You can also contact the facility directly for more information.
Different gyms have different rules. These rules apply to all members, not just Fitness Passport.
What is the process if I decide to cancel?
There is a 30-day notice period required for membership cancellation requests. When you sign up with Fitness Passport you join for a minimum 12-month contract period. If you elect to withdraw from the program inside that 12-month period, you may be charged a cancellation fee (see matrix below).
However, if you leave Monash Health and are no longer eligible for Fitness Passport, you can cancel your membership at no cost.

Will you advise me when new facilities come on board? My gym is not available yet.
Fitness Passport is always working to add new gyms, pools and leisure facilities. Keep an eye on the Fitness Passport map for new locations close to you.
Can my family use the HealthWise Gym at Monash Medical Centre if I take out a family membership?
Due to the location of HealthWise, only Monash Health employees will be able to access the gym.
I am a member of a gym that is now on Fitness Passport and still have several months left on my annual membership. Can I transfer now to Fitness Passport?
Talk to your gym to find out if you’re able to leave your membership early and register for Fitness Passport.
I contacted a gym on the list of participating facilities and was told that they are not part of Fitness Passport. What should I do?
Please email to the Wellbeing Team. We’ll liaise with Fitness Passport to confirm your gym is part of the program.
