Medication Lists just got friendlier

Thanks to an upgrade of the EMR, we have new simplified medication lists replacing the Merlin manually-generated patient medication list usually provided to patients by Pharmacists on discharge.

The original Medication List is still present in the Doctor’s Discharge Summary.

Using this new EMR function improves outcomes for our patients by helping them understand their ongoing care after they’ve left the hospital.

How to create a PFML

Once you have finalised a patient’s care plan using the EMR Discharge Reconciliation function, the EMR will generate a PFML that is easy for patients and other care providers to understand.

Medical & Pharmacy staff must use the Discharge Medication Reconciliation function in the EMR to produce an accurate PFML.
