Each year, Monash Health brings awareness to the importance of medication safety practices to reduce the risk of errors and improve patient safety and outcomes.
This year we celebrate Medication Safety Week from Monday 11 to Friday 15 September.
Unsafe medication practices and errors can lead to avoidable harm for our patients resulting in additional treatments, monitoring requirements and increased length of stay.
At Monash Health we strive for medication safety for every medicine, every time.
- Ensure you review your patient’s medication administration record (MAR) or medication chart regularly.
- Can any suspended/withheld medicines be recommenced?
Apply the 6 rights of medication safety every time you order/prescribe, dispense/supply or administer medicines:
- Right patient
- Right medication
- Right dose and formulation
- Right time
- Right route
- Right reason
