Join your colleagues and complete the Employee Workplace Declaration form today

Last week we sent out the Employee Workplace Declaration form and had a great response. However, we require all employees to complete the form. If you have not completed the form yet, you must do so by Friday 28 August. Click here to access the form. This information will help inform the Department of Health and Human Services response to COVID-19 outbreaks.

This declaration includes any paid, unpaid or voluntary work you do for any organisation, including Monash Health. Examples of other workplaces may include pharmacies, retail stores, hospitality services, community centres or sport centres. For more information, please read the FAQs.

Please note, if you are Senior Medical Staff you are not required to complete this form, as you would have completed the Workplace Mobility Survey.
