Congratulations to our STAR Award winners for Exceptional Teamwork

A big congratulations to the Monash Children’s at Home team for winning the Exceptional Teamwork STAR Award. 

The STAR Award for Exceptional Teamwork is given to a team who builds an inclusive, high-performing culture, work together to improve systems, processes and the work environment, and put quality and kindness at the heart of everything they do. 

Jackie Hopley, Nurse Manager, is here to accept the award on behalf of the team. 

“Caring for a patient in their home is such a privilege,” says Jackie. “At Monash Children’s at Home, we pride ourselves of providing the highest standard of care for our families. We always try to keep the patient and family at the centre of everything we do.” 

The team were nominated for working together to solve a difficult situation for one of our young patients. 

The patient had been in hospital for a month and was desperate to get home. Although the patient was clinically stable for discharge, they needed 2 further weeks of intravenous antibiotics. Unfortunately, the family lived in Jindivick, outside the catchment area for Hospital in the Home.  

To get the young patient home safely, the team sprang into action. They taught the father how to change the Baxter bottle with the antibiotics himself. All the supplies the family needed were sent home with them on discharge, including enough Baxter bottles. 

One of our HITH nurses went to visit the family on the first day after discharge, to supervise the first scheduled Baxter bottle. Subsequent sessions were held via telehealth to support the father. 

Thinking outside the box led to a great outcome for the patient and the family.
