The Employee Workplace Declaration form is a critical safety initiative that is based on a Directive from the Public Health Commander. This information will help the timely, effective management of potential outbreaks between workplaces.
There has been a great response, with 63% of employees completing the form. However, the Department of Health and Human Services requires 100% completion.
If you have not completed the Declaration form, please take five minutes to do so now. You can access the form here.
If you manage a team, please log in to the Business Intelligence Portal to check the status of your team members and encourage those who have not declared, to complete the form today.
Students undertaking placements with Monash Health have also been asked to complete a declaration form.
