Our STAR Award winner for Improving Care and Access is Dione Belbin, Administration Assistant with the Transition Care program at Kingston centre.
This award is for an individual, group or team that develops improved ways for patients to access and manage their care.
Dione’s team at the Transition Care program say they couldn’t function without her hard work and quiet achievement behind the scenes.
Dione is incredibly knowledgeable and always takes the initiative to support the best possible outcomes for the Transition Care program’s clients.

“We’re a team that look after clients who might have had a long stay in hospital,” says Dione. “Or who might need some extra help in for their independance transitioning back home.”
Late one Friday afternoon, Dione received a phone call from a residential care facility letting her know that a client had never arrived. The client was meant to have been collected at 10am from home by a private provider to take him to the Facility where he was being admitted as a permanent resident.
Despite it being past home time for Dione, she acted quickly. First, she looked up the client details and used her detective skills to work out what had gone wrong with the client transfer. She took the initiative to resolve the problem with the provider that was meant to transport the client, by raising it as an urgent issue.
Thanks to Dione’s work ethic and quick action, the client’s safe travel to the facility was achieved, after some additional support from other team members to ensure that he would have enough medication to last the long weekend.
“It does make me feel honour that the team have recongised me,” says Dione.
