If you have received your influenza vaccine outside of Monash Health, it is important that you fill out the online declaration form to tell us that you have received your flu vaccination elsewhere this year.
If you are not planning on receiving the influenza vaccine, you still need to fill out the form and let us know you are not participating. Declaration is mandatory.
The form takes less than one minute to complete, all you need is your Monash Health employee ID number.
Declare todayIf you have not received your flu vaccination for the 2023 season, we urge you to get vaccinated to get in front of the flu now. This month, ongoing vaccination for Influenza will become limited. You can find opportunities to get vaccinated for Influenza for free here.
You can receive your COVID-19 vaccine at the same time as your Influenza vaccination at Monash Immunisation, Monash Medical Centre.
Free employee flu vaccination will continue to be available at Infection Prevention Offices at specific times and Monash Immunisation at Monash Medical Centre, Clayton.
Please note that the cost of flu vaccination outside of our employee program will not be covered by or automatically reported to Monash Health.
